To qualify for the rebate:

Do Online Purchases Qualify for the CooperVision rebate?
No. In theory, it’s possible to get the rebate for an online purchase as long as your eye exam receipt matches the retailer that you purchased your eye exam from (according to their customer service hotline). However, in practice most online eye exams are not handled by the retailers so the receipts won’t match. For example, I recently took a free online eye exam from a major retailer and the name on the invoice didn’t match the website.
The good news is that online contact lens prices can be much cheaper than in-store prices so you might still save more money by buying online at the lowest price possible.
